Charlotte's Cat Shack Cat Boarding Contract

We are an “Indoor cat,” overnight boarding service. The location is in a private residence that is licensed and insured. Pre-boarding visits, drop-off, and pick-up are by appointment only.

We do not board cats that are allowed to roam outdoors, unaltered males, kittens under 6 months of age, and “specialty care.” (Diabetic, epileptic and asthmatics, “fragile” seniors, seriously ill cats, unstable conditions, (heart and kidney), or cats displaying major potty issues. Any services or fees will be mutually agreed upon.

Current Vaccines are required. If you request a waiver for a vaccine, a letter of explanation is required from your veterinarian. NO exceptions. (Washington State introduced the Rabies vaccine as a new law in 2012).

Flea preventative is required. If fleas are observed on your cat/bedding and if pre-boarding flea preventative has been applied longer than 30 days ago, I reserve the right to apply new dose for a fee. If fleas are observed in general, I will apply a powder to bedding.

Pets experiencing any sneezing, coughing, or “cold type,” symptoms, will NOT be accepted. Feline respiratory virus can be serious and is highly contagious. The required FVR-CP vaccine helps protect against this.

If any contagious issues are suspected, it will be addressed either with a temporary OTC medicine, or vet visit depending on the issue and severity. (For example Ear mites or Ringworm). Very susceptible cats are any newly adopted or purchased animal. If your pet is contagious, you or your representative will be notified to pick your pet up. While waiting for pick up, your pet will be isolated.

Any prescriptions are to be discussed before actual stay and if approved, the instructions are to be provided along with medications in their original container(s), with any special dosing equipment, if applicable.

I am given permission by you, to act as your agent to have your pet or pets, treated by a licensed veterinarian, vet tech, and staff, in case of emergency or illness, decided to be necessary by me, and to follow your wishes in the boarding file, or your emergency contact regarding treatment and costs. You allow me to transport your pet or pets, to such a treatment facility, as long as they are in my care, and you will be responsible for all veterinary fees. If it can be arranged, the emergency contact will take pet/pets to the vet.

You will not hold me or Charlotte’s Cat Shack Cat Boarding liable for issues resulting from inaccurate vaccine dates, boarding stress, common viruses or issues that my pet(s) may carry prior to boarding, applications of flea preventative/treatment, or over the counter medications that we have agreed upon.

If I cannot successfully contact you, or your emergency contact, or get information from the pet’s vet, in the event the pet or pets are not picked up in a reasonable time period after the end of stay, I have the right to surrender pet or pets, to a facility for holding or adoption of my choice. After all attempts are exhausted to make all contacts, the pet’s vet will be notified and all information will be posted on my website.

In the event of any natural disaster there are special organizations in place to reunite pets with their pet parent’s, so please keep contact information in veterinarian files, or any other files up to date!

Photos will be taken of your pet(s) for their boarding file, and my website, solely for advertising purposes. All policies and information is listed on website, and copies are available on request.

This form can be submitted online and I will have it ready and printed for you sign upon your arrival.
